Child and Youth Health Network for Eastern Ontario

Profile:

Child and Youth Health Network for Eastern Ontario (CYHNEO) has created a  document entitled "Using Population Health Data to Profile the Health and Well-Being of Children and Youth in Eastern Ontario"

This Working Document was created in response to the identified need for a tool to help agencies use a population health approach for planning services for children and youth. The report, Using Population Health Data to Profile the Health and Well-being of Children and Youth in Eastern Ontario (Profile), provides:

  • a template that addresses key categories of population health
  • indicators and related data to populate these categories
  • data for the Champlain region and for each of the five counties in Eastern Ontario
  • comparisons across counties, the region and Ontario

The document is available for download on their website.

Partnership:

Many of the indicators and data provided in the report have been incorporated and mapped into the CIMS. The CIMS complements the CYHNEO's profile by allowing agencies to visualize the data spatially at different levels of geography and to bring their own data into a secure system and combine their service or client data with population health data that can then be mapped as an aid for community-based research, planning and information sharing.

Atlas

The Atlas provide maps on some of the key aspects of the report to complement the data available in the report.

The Atlas is available in two formats: a paper format available for download and an interactive format that can be access through the CIMS portal: http://cims.mapbase.ca/viewer/.

 Presentations and Workshops:

The CYHNEO and the CIMS staff are organizing joint presentations/workshops about the profile and the CIMS tool through Eastern Ontario.
